Overview

To promote expert, individualised patient centred clinical care for clients with compromised skin integrity and pressure ulcers by using evidence based practice for the population of Croydon Health Services (CHS). The pressure ulcer nurse will assess and plan care for patients referred to the service and work in conjunction with the tissue viability team. The role will include the provision of care and supervision of the delivery of this care by clinical staff. The post holder will offer education and support to patients, carers and other health care professionals. The pressure ulcer nurse will work closely with other health care professionals to promote a high standard of care in the prevention and management of pressure ulcers ensuring the pressure ulcer policy & pathway are followed. This is in line with local and national guidelines and best practice in the aim to reduce all grades of pressure ulceration.

Role and duties

District Nurse / Caseload Holder - Waddon Clinic

  • Provide efficient and effective nursing care to patients in their own homes within integrated care teams.
  • Lead and supervise delivery of care by clinical staff and ensure adherence to the pressure ulcer policy & pathway.
  • Offer education and support to patients, carers and other health care professionals.
  • Work in collaboration with the tissue viability team and other health care professionals to promote best practice in prevention and management of pressure ulcers.

Requirements

  • Band 6
  • Registered Nurse with a Specialist Practice Qualification in District Nursing or experience working as part of the Community Nursing Team.
  • All candidates must have a clean full UK driving licence and the use of a car for work purposes.
