Active Lives Service β Senior Community Support Worker x 2

Active Lives supports adults with disabilities to live the life they choose by working in a person centred way offering support that is aligned to the values of Community Led Support.
In this role you will work with adults with learning disabilities and autism and other complex needs; delivering sessions that build skills and support emotional and physical wellbeing. Our sessions take place in one of our Hubs and the local community.
Our ethos is to support people to be as independent as they can be in all areas of their life, to be actively involved and included in their local communities and to reduce or avoid people becoming building dependant. Learning opportunities range from Pathways to Employment to supporting people using a small/next steps approach to develop life skills and communicate their wants and needs so that they can make choices about their lives.
The successful candidate will deliver sessions and provide professional support and guidance to a team of Community Support Workers, working with and to the Team Leader. They will have high professional standards and be a good team player, they will have knowledge and experience of positive behaviour support, have skills that enable communication and be able to think creatively to design and deliver sessions that meet individual person centred outcomes. They will also have experience of delivering independence outcomes in the community including in people\βs homes.
Initially this role will be based at Whitehorse Hub and The Cherry Hub however, like all roles in Active Lives the location and team may change according to service demand and evolution.
