Croydon Council is seeking an Experienced Social Worker to join the Transitions Service for young adults aged 18-25 with disabilities, autism, learning disabilities, mental health needs and complex needs as they move into adulthood. You will undertake strengths-based assessments, care and support planning, reviews and safeguarding work in line with the Care Act 2014 and Mental Capacity Act 2005.
