Team Manager – Adult Social Care Transitions Grade 15
Salary range: £55,323 – £57,404 per annum plus fantastic benefits.

An exciting opportunity has arisen for the Team Manager role in the 25‑65 Disability Service. You will lead a dedicated team focused on promoting independence and empowerment for vulnerable adults with learning disability, physical disability, multiple diagnosis/complex needs (including mental health and learning disability). Your work will have a direct impact on how needs are met and shape service delivery. You will drive positive change in collaboration with internal and external partners such as the ICB, local GPs, voluntary organisations, and other stakeholders.
As a leader, you will support Advanced Social Workers, Experienced Social Workers, and Health and Wellbeing Workers, helping them provide tailored support through assessments and plans under the Care Act. You will guide the team to creatively use resources, including technology and community assets, to enable our clients to live independently.
Hybrid working from home and from the office is in place, with an office presence two‑to‑three days a week.
Main Responsibilities Day‑to‑day responsibility for a team of social workers and Health and Wellbeing Workers.
Support the investigation of complaints at stage 1 where concerns are not complex.
Ensure that Care Act and Mental Capacity Act related assessments and decisions are carried out to a high quality and comply with relevant policies and procedures, taking appropriate action.
Be responsible for the quality assurance of statutory documents within delegated responsibility, ensuring appropriate and proportionate records and documentation are completed timely.
Line‑manage Advanced & Experienced Social Workers.
Support continued development of the team, driving best practice.
Drive and support key performance indicators.
Liaise with mid‑ and senior‑management to implement and deliver agreed actions for the benefit of clients, stakeholders and local authority.
Ideal Candidate High motivation and flexible, with a proven track record of supporting and leading a team.
Sound knowledge and experience of current legislation and its application in a social care setting.
Passion about ensuring a smooth and timely transition for vulnerable adults.
Applied knowledge of the Care Act and Mental Capacity Act, with experience in complex safeguarding and professional judgement.
Excellent communication skills, able to provide professional, reflective supervision, guidance, and advice to front‑line supervisors, social workers, and family support workers.
