Health and Wellbeing Assessor
Salary range: Grade 6 £35,520–£36,585 p.a. plus fantastic benefits.
Contract: Fixed Term – 31 March 2027 | Hours: 36 hours per week | Location: Croydon.
About the Role
As a trusted assessor, you will support individuals to regain independence and live confidently at home. You will work alongside Social Workers, Occupational Therapists, Physiotherapists, and Intermediate Care Nurses, delivering person‑centred support across hospitals, community settings, and people’s homes.
Responsibilities
- Assess clients’ health and wellbeing needs.
- Develop and implement individualised support plans.
- Provide person‑centred support that empowers clients to reach their full potential.
- Collaborate with multidisciplinary teams to achieve optimal outcomes.
- Maintain accurate records and documentation of activities and progress.
- Supervise and support team members as required.
