Salary range: £53,172 – £55,323 p.a. plus competitive benefits.

The Role: Advanced Social Worker (LIFE Team)

This fast-paced role focuses on hospital discharge within the Discharge to Assess (D2A) model. You will support individuals in returning home safely, delivering strength-based assessments that promote independence and recovery.

Key Responsibilities

  • Lead and deliver high-quality, strength-based assessments in the community.
  • Support hospital discharge pathways through short-term, outcome-focused interventions.
  • Provide supervision, mentoring, and guidance to Social Workers and Health & Wellbeing Assessors.
  • Participate in the Duty Senior rota, allocating work and offering professional advice.
  • Work within a multi-disciplinary, integrated team to deliver person-centered care.
  • Contribute to service development and continuous improvement.

About You

  • Qualified Social Worker with post-qualifying experience.
  • Confident in supervision and supporting staff development.
  • Experience in adult social care practice, ideally including discharge or reablement.
  • Skilled in strength-based practice and person-centered approaches.
  • Able to work flexibly in a fast-paced, evolving environment.
  • Collaborative team player with professionalism, creativity, and drive.
