Crown House Surgery is currently seeking an experienced Clinical Pharmacist holding a prescribing qualification. The role is part time of 22 hours per week, split over 3 days. The successful candidate will be passionate and enthusiastic, well organised, highly motivated and skilled in their role, who will work autonomously within their scope of practice. Supporting the practice with improvements to maximise safe, cost effective best practice in prescribing to improve the quality of patient care within the practice and PCN.

You will support all aspects of medicines management within the practice, including medication reviews, medicines reconciliation, structured medication reviews, the management of systems to ensure safe prescribing and offer support to the wider practice team. You will also manage medication queries and repeat prescribing systems in the practice. In this role you will be supported by our Lead Clinical Pharmacist and Clinical Lead GP Partner. You will work within your competence to deliver a service and manage the health and social care needs of patients within the practice. Applicants must hold relevant qualifications with their own professional bodies. A strong commitment to personal and professional development and teamwork are essential for this role.

Main duties of the job

  • Working within our established & experienced pharmacy team, to ensure patients receive a high level of care and service delivery.
  • To work as part of the primary care team in the practice supporting the team to help achieve efficiencies & effectiveness.
  • To support with leading improvements to maximise safe, cost effective best practice in prescribing to improve the quality of patient care within the practice.
  • Registered with General Pharmaceutical Council.
  • Actively take a personalised care approach and population centred care approach to enable shared decision making.
  • Committed to delivering high standards of patient care.

About us

Crown House Surgery is a well established, forward thinking & supportive GP partnership of 7 GPs, made up of a friendly team providing care to over 11,600 patients. We are part of Retford & Villages PCN, the role will be employed by Retford & Villages PCN via Fidelium Health (FHL), the position is fully embedded, and HR Management will be at Crown House Surgery. Your contract of employment will align with the policies and procedures of this practice.

The practice has a wide multidisciplinary team, with a nursing team with expertise in chronic disease and treatment room skills and other Allied Health Professionals including another Clinical Pharmacist, Pharmacy Technicians, Advanced Clinical Practitioners and a Social Prescriber. We are committed to delivering high quality care with a focus on continuity. We operate a usual doctor system, however our expectation is for the wider clinical workforce and usual doctor to offer continuity of care with a team level approach.

Experience

  • Has previously worked in a primary care setting or a Primary Care Network.
  • Able to analyse and interpret prescribing data.
  • Ability to listen, empathise with people and provide person centred support in a non-judgemental way.

  • Courteous, respectful and helpful at all times.

  • Ability to work flexibly and enthusiastically within a team or on own initiative.
  • Ability to communicate complex and sensitive information effectively with people at all levels by telephone, email and face to face.

  • Knowledge of and ability to work to policies and procedures, including confidentiality, safeguarding, lone working, information governance, and health and safety.

  • Current full driving licence and sole use of a car.

Qualifications

  • Completion of an undergraduate degree in Pharmacy, and registration with the General Pharmaceutical Council.
  • Extensive knowledge of medicines optimisation, acquired through post graduate clinical pharmacy diploma or equivalent experience or training.
  • Proven clinical pharmacy knowledge and evidence of commitment to continuing education.
  • Good clinical IT system knowledge.
  • Successful completion or working towards the CPPE Primary Care Pharmacy Education Pathway.
  • Knowledge of other IT systems, including ability to use word processing skills, emails and internet based applications.
