Crown House Surgery is currently seeking an experienced Clinical Pharmacist holding a prescribing qualification. The role is part time of 22 hours per week, split over 3 days. The successful candidate will be passionate and enthusiastic, well organised, highly motivated and skilled in their role, who will work autonomously within their scope of practice.

Supporting the practice with improvements to maximise safe, cost effective best practice in prescribing to improve the quality of patient care within the practice and PCN. You will support all aspects of medicines management within the practice, including:

  • Medication reviews
  • Medicines reconciliation
  • Structured medication reviews
  • The management of systems to ensure safe prescribing
  • Offer support to the wider practice team

You will also manage medication queries and repeat prescribing systems in the practice. In this role you will be supported by our Lead Clinical Pharmacist and Clinical Lead GP Partner. You will work within your competence to deliver a service and manage the health and social care needs of patients within the practice. Applicants must hold relevant qualifications with their own professional bodies. A strong commitment to personal and professional development and teamwork are essential for this role.
