Crown Heights Medical Centre is seeking an experienced Practice Nurse with a strong interest in diabetes management to join our clinical team following the retirement of a long-standing member of staff. This is an excellent opportunity for a motivated nurse to play a key role in delivering high-quality diabetes care within primary care, supporting patients with long-term condition management and contributing to improved health outcomes. The successful candidate will be confident managing diabetes reviews and long-term condition monitoring, working closely with the wider multidisciplinary team including GPs, pharmacists and healthcare assistants.
Main duties of the job
- Running diabetes clinics and annual reviews
- Supporting management of patients with Type 1 and Type 2 diabetes
- Monitoring blood results including HbA1c, renal function and lipid profiles
- Supporting medication reviews and optimisation of diabetes treatments
- Providing lifestyle advice and patient education to support self-management
- Supporting insulin initiation and titration where appropriate
- Identifying patients requiring escalation or specialist referral
- Supporting the practice in achieving QOF targets for diabetes
- Contributing to clinical audits and quality improvement
- Working collaboratively with the multidisciplinary team including pharmacists and GPs
The role may also include broader practice nursing duties depending on experience, such as chronic disease reviews, vaccinations and health promotion.

Person Specification
- Registered Nurse (NMC registered)
- Experience working in primary care or long-term condition management
- Experience managing patients with diabetes
- Strong communication and patient education skills
- Ability to work independently and as part of a multidisciplinary team
- Experience supporting insulin initiation or titration
- Experience managing other long-term conditions (e.g. hypertension, asthma, COPD)
- Experience using EMIS clinical system
