A Day Habilitation Coordinator is responsible for coordinating and overseeing individuals’ program plans through the development and evaluation of goals, objectives, and behaviour or mental health plans, as well as coordinating services with other providers and facilitating regular interdisciplinary meetings. The Day Habilitation Coordinator directly supervises Direct Support Professionals (DSPs) and works closely with their immediate supervisor, the Day Habilitation Manager, to monitor progress on each individual program plan and audits documentation to ensure Day Habilitation compliance standards are met and present for accurate and timely billing.
Essential Duties and Responsibilities
- Coordinate, develop, and monitor assigned individual program plans and behaviour support programs where applicable.
- Implement, coordinate, and facilitate Interdisciplinary Team (IDT) meetings annually and as needed throughout the service plan year.
- Maintain professional and collaborative relationships with clients’ IDT members.
- Directly supervise designated Lead Direct Support Professionals as assigned, including but not limited to, timecard oversight, PTO review, and performance.
- Advocate for needed services and programming for the assigned client caseload.
- Maintain Safety Care Specialist certification; coach direct reports to use agency approved behavioural intervention techniques.
- Maintain current and accurate records for assigned caseload.
- Participate in the Program Quality Review process to ensure accuracy and quality of client files.
- Maintain up to date knowledge of changes in Home and Community Based Services (HCBS) and the Commission on Accreditation of Rehabilitation Facilities.
- Audit Day Hab documentation for accuracy, ensure compliance with HCBS guidelines, and submit billing to the accounting department by established deadlines.
- Communicate program needs directly to the Day Hab Manager.
- Attend all required agency meetings, trainings, and participate in committees designated by Leadership.

Minimum Qualifications
- Training: Able to obtain and maintain certification in Medication Management, CPR, and Safety Care within the first 6 months of hire.
- Education: Bachelor’s degree preferred or equivalent combination of education and experience.
- Experience: One year of experience working with people with disabilities. Strong communication skills and people skills.
- Ability to work independently and as part of a team.
- Ability to develop and implement CRI PSCPs in a non-biased, accurate, and client-centered manner.
- Interact in an empathetic and professional manner with clients.
- Always maintain confidentiality in all aspects of this position.
- Ability to maintain accurate and up-to-date records.
- Valid driver’s license, current auto insurance, and reliable transportation.
- Pass required background checks with no evidence of fraud, waste, abuse and cleared to work by the Iowa Department of Health and Human Services.
- Ability to operate a variety of office equipment to include but not limited to computers, printers, and scanners.
- Working knowledge of Microsoft Office Suite software.
Additional Qualifications
- Valid driver’s license and an acceptable driving record in accordance with risk management standards.
- Ability to travel extensively throughout the community.
- Physical ability to assist with lifting individuals when required.
- Clear and effective oral and written communication skills.
