This post will be based at Crossfell Health Centre, part of the Greater Middlesbrough Primary Care Network. We are looking for a dynamic pharmacist to join our highly skilled and experienced team to deliver medicines optimisation services.
Contracted hours of the role will be Monday-Wednesday 08:30-16:30.
Main duties of the job:- Provide medicines optimisation support and expert pharmaceutical advice.
- Implement and deliver prescribing initiatives, improve prescribing safety, and conduct medication reviews.
- Work as part of a multi-disciplinary team in a patient-facing role to clinically assess and treat patients.
- Be responsible for the care management of patients with chronic diseases and undertake clinical medication reviews.
- Provide leadership on person-centred medicines optimisation and quality improvement.
- Develop relationships and work closely with other pharmacy professionals across the Primary Care Network.
- Undertake a central role in the clinical aspects of shared care protocols and clinical research.
- Recognise the roles of other colleagues within the organisation and their role in patient care.
- Demonstrate appropriate communication to gain cooperation from relevant stakeholders.
- Actively work toward developing and maintaining effective working relationships.
- Minimum of 3 years post-graduate experience in pharmacy.
- Holds or is working towards an independent prescribing qualification.
- Able to plan, manage, monitor, advise, and review general pharmaceutical care programmes.
- Involves patients in decisions about prescribed medicines and supports adherence as per NICE guidelines.
- Demonstrate understanding of the pharmacy role in governance.
- Contribute to the workplace vision and improve quality within service limitations.
- Demonstrates self-development through continuous professional development activity.
- Engaged with the review and appraisal systems within the practice.
- Comply with the Practices Health and Safety policies.
- Report incidents using the organisations Incident Reporting System.
- Co-operate with all policies and procedures designed to ensure equality of employment.
- Respect patient confidentiality at all times.
