This post will be based at Crossfell Health Centre, part of the Greater Middlesbrough Primary Care Network. We are looking for a dynamic pharmacist to join our highly skilled and experienced team. An exciting opportunity has arisen to develop your skills in primary care.

Contracted hours of the role will be Monday-Wednesday 08:30-16:30.

Main duties of the job:

The role offers a varied work agenda providing medicines optimisation support and expert pharmaceutical advice, including:

  • Implementation and delivery of prescribing initiatives
  • Improving prescribing safety
  • Provision of medication reviews

The successful applicant should demonstrate excellent clinical and communication skills, with experience in:

  • Implementing medicines optimisation
  • Quality and patient safety initiatives
  • Resolving prescribing issues

You will be a Registered Pharmacist with the General Pharmaceutical Council, preferably an Independent Prescriber. You must also be computer literate, organised, and able to work independently as well as part of a team. The successful candidate will be able to prioritise workload, build key relationships, and confidently interact with a wide range of stakeholders such as GPs, nurses, secondary care clinicians, and patients.

Crossfell Health Centre has 9000 patients, 2 GP Partners, 5 salaried doctors, 3 nurses, 2 HCAs, a General Practice Assistant, and a team of receptionists, secretaries, and admin staff.

Primary Duties and Areas of Responsibility:
  • Work as part of a multi-disciplinary team in a patient-facing role to clinically assess and treat patients using expert knowledge of medicines for specific disease areas.
  • Be a prescriber and work alongside the general practice team.
  • Manage care for patients with chronic diseases and undertake clinical medication reviews.
  • Provide specialist expertise in the use of medicines while addressing public health and social care needs.
  • Deliver structured medication reviews to support patients in managing their medications.
  • Support integration of general practice with wider healthcare teams.
  • Develop relationships with other pharmacy professionals across the Primary Care Network.
  • Participate in clinical aspects of shared care protocols and clinical research.
Collaborative Working Relationships:
  • Recognise the roles of colleagues within the organisation.
  • Use appropriate communication to gain cooperation from relevant stakeholders.
  • Develop and maintain effective working relationships both within and outside the practice.
  • Liaise with ICB Medicines Optimisation Team on prescribing matters.
Knowledge, Skills and Experience Required:
  • Experience/awareness of common long-term conditions.
  • Minimum of 3 years post-graduate experience in pharmacy.
  • Working towards an independent prescribing qualification.
  • Able to plan, manage, monitor, advise, and review pharmaceutical care programmes.
  • Accountable for delivering professional expertise and direct service provision.
Leadership:
  • Understand the pharmacy role in governance and implement it appropriately.
  • Contribute to the workplace vision and improve quality within service limitations.
Management:
  • Understand national priorities for the team/service.
  • Utilise resources effectively and conform to relevant standards of practice.
Education, Training and Development:
  • Demonstrate characteristics of a role model.
  • Engage in continuous professional development.
  • Ensure appropriate clinical supervision is in place.
Research and Evaluation:
  • Critically evaluate and review literature.
  • Identify gaps in the evidence base to support practice.
Health and Safety/Risk Management:

Comply with the Practices Health and Safety policies and report incidents using the organisations Incident Reporting System.
