This post will be based at Crossfell Health Centre, part of the Greater Middlesbrough Primary Care Network. We are looking for a dynamic pharmacist to join our highly skilled and experienced team to deliver medicines optimisation services.
Contracted hours of the role will be Monday-Wednesday 08:30-16:30.
Main duties of the job:- Provide medicines optimisation support and expert pharmaceutical advice.
- Implement and deliver prescribing initiatives, improve prescribing safety, and conduct medication reviews.
- Demonstrate excellent clinical and communication skills.
- Be a Registered Pharmacist with the General Pharmaceutical Council, preferably an Independent Prescriber.
- Work independently and as part of a team, prioritising workload and building key relationships.
Crossfell Health Centre has 9000 patients, 2 GP Partners, 5 salaried doctors, 3 nurses, 2 HCAs, a General Practice Assistant, and a team of Receptionists, Secretaries & Admin.
Primary Duties and Areas of Responsibility:- Work as part of a multi-disciplinary team in a patient-facing role.
- Be responsible for the care management of patients with chronic diseases.
- Provide specialist expertise in the use of medicines.
- Deliver a programme of structured medication reviews.
- Support further integration of general practice with wider healthcare teams.
- Develop relationships with other pharmacy professionals across the Primary Care Network.
- Recognise the roles of colleagues within the organisation.
- Use appropriate communication to gain cooperation from stakeholders.
- Develop and maintain effective working relationships.
- Minimum of 3 years post-graduate experience in pharmacy.
- Holds or is working towards an independent prescribing qualification.
- Able to plan, manage, monitor, advise, and review general pharmaceutical care programmes.
- Accountable for delivering professional expertise and direct service provision.
- Understand the pharmacy role in governance.
- Contribute to the workplace vision.
- Motivate self to achieve goals.
- Understand national priorities for the team/service.
- Utilise resources effectively.
- Demonstrate self-development through continuous professional development activity.
- Keep up to date with relevant clinical practice.
