This post will be based at Crossfell Health Centre, part of the Greater Middlesbrough Primary Care Network. We are looking for a dynamic pharmacist to join our highly skilled and experienced team. An exciting opportunity has arisen to develop your skills in primary care.
Contracted hours of the role will be Monday-Wednesday 08:30-16:30.
Main duties of the job:The role offers a varied work agenda providing medicines optimisation support and expert pharmaceutical advice, including:
- Implementation and delivery of prescribing initiatives
- Improving prescribing safety
- Provision of medication reviews

The successful applicant should demonstrate excellent clinical and communication skills, with experience in implementing medicines optimisation, quality and patient safety initiatives, and resolving prescribing issues. You will be a Registered Pharmacist with the General Pharmaceutical Council, preferably an Independent Prescriber. You must also be computer literate, organised, and able to work independently as well as part of a team.
Key responsibilities include:
- Working as part of a multi-disciplinary team in a patient-facing role to clinically assess and treat patients using expert knowledge of medicines for specific disease areas.
- Being a prescriber and working alongside the general practice team.
- Managing care for patients with chronic diseases and conducting clinical medication reviews.
- Providing specialist expertise in the use of medicines while addressing public health and social care needs.
- Delivering structured medication reviews to support patients in managing their medications.
- Supporting integration of general practice with wider healthcare teams.
- Developing relationships with other pharmacy professionals across the Primary Care Network.
Knowledge, Skills and Experience Required:
- Experience/awareness of common long-term conditions.
- A minimum of 3 years post-graduate experience in pharmacy.
- Working towards an independent prescribing qualification.
- Able to plan, manage, monitor, advise and review general pharmaceutical care programmes.
- Accountable for delivering professional expertise and direct service provision.
Leadership and Management:
- Understanding of the pharmacy role in governance.
- Ability to improve quality within service limitations.
- Understanding of national priorities for the team/service.
Education, Training and Development:
- Demonstrates self-development through continuous professional development activity.
- Engaged with review and appraisal systems within the practice.
Health and Safety/Risk Management:
- Compliance with Health and Safety policies.
- Adherence to the Data Protection Act and Access to Health Records Act.
