Cromwell Medical Staffing is currently seeking an experienced Band 6 RMN to support community mental health services on a 12-week placement. This is an excellent opportunity for a skilled mental health nurse to manage a defined caseload, deliver high-quality, patient-centred care, and contribute to effective community-based mental health interventions.

Key Responsibilities

  • Assess, plan, implement, and evaluate care for a caseload of service users experiencing a range of mental health conditions
  • Undertake comprehensive mental health and risk assessments, ensuring appropriate risk management plans are in place
  • Act as Care Coordinator in line with CPA (Care Programme Approach) requirements
  • Develop and deliver person-centred care plans in collaboration with service users, carers, and the multidisciplinary team
  • Participate in the Duty Worker and Initial Assessment rota, providing timely assessments and interventions
  • Support safe and effective discharge planning from inpatient services and facilitate smooth transfers of care.

Requirements

  • NMC Registered Nurse Mental Health (RMN)
  • Minimum 6 months recent experience working as a Community Psychiatric Nurse
