Description:
We are currently recruiting experienced Band 6 Community Psychiatric Nurses (CPNs) to join the Recovery Wellbeing Team at the Taylor Centre in Southend. This is an excellent opportunity for skilled mental health professionals who are experienced in community mental health and care coordination.
Key Responsibilities:
- Manage a caseload of up to 35 patients with severe and enduring mental health conditions
- Act as Care Coordinator in accordance with the Care Programme Approach (CPA)
- Complete comprehensive mental health assessments and risk assessments
- Develop, implement, and review person-centred care plans
