Description:
We are currently recruiting experienced Band 6 Community Psychiatric Nurses (CPNs) to join the Recovery & Wellbeing Team at the Taylor Centre in Southend. This is an excellent opportunity for skilled mental health professionals who are experienced in community mental health and care coordination.
Key Responsibilities:
- Manage a caseload of up to 35 patients with severe and enduring mental health conditions
- Act as Care Coordinator in accordance with the Care Programme Approach (CPA)
- Complete comprehensive mental health assessments and risk assessments
- Develop, implement, and review person-centred care plans
