As a Clinical pharmacist you will provide a comprehensive and patient centred Clinical Pharmacy service to patients at Bupa Cromwell Hospital including participation in the antibiotic stewardship round, MDT input, therapeutic drug monitoring, medicines reconciliation and dispensary support. You will be responsible for delivering safe and effective pharmaceutical care and best medicines management practice.
Main duties of the job:
- Providing a comprehensive and patient centred Clinical Pharmacy service to patients at Bupa Cromwell Hospital including participation in the antibiotic stewardship round, therapeutic drug monitoring, medicines reconciliation and dispensary support.
- Facilitating and promoting rational and optimal medicines usage.
- Providing assimilated and relevant medicines information to healthcare professionals and patients.
- Perform regular monitoring of medicines management on wards/departments including Controlled Drugs and advise nursing staff on legal control, safe custody and security of drugs.
- Validate prescriptions in the dispensary to ensure appropriateness for the patient in accordance with procedure and good practice.
- Provide relevant medication counselling to patients to enable them to take their medicines safely and effectively.
- To participate in ward rounds, clinical hand-over and promote the role of the Pharmacist as an integral part of the hospital multi-disciplinary clinical team.
