An exciting opportunity has arisen for an enthusiastic, experienced andhighly motivated Prescribing Practice Nurse with a Diploma in Diabetes to joinour friendly, supportive and well-organised practice.
We are looking for a dedicated clinician who is passionate aboutdelivering high-quality patient care and has the skills and confidence tomanage patients with diabetes and other long-term conditions independently. Thesuccessful candidate will work as part of our established multidisciplinaryteam, helping us provide excellent care to our patient population ofapproximately 11,000 patients across two sites.

Main duties of the job
The successful candidate will:
- Lead and manage diabetes clinics, providing comprehensive assessment, treatment and ongoing management of patients with diabetes.
- Independently prescribe within their scope of practice.
- Support the achievement of Quality and Outcomes Framework (QOF) indicators and local quality initiatives.
- Undertake reviews for patients with long-term conditions, including diabetes, hypertension, cardiovascular disease and respiratory conditions where appropriate.
- Provide evidence-based advice on lifestyle modification, self-management and preventative healthcare.
- Review pathology results and adjust treatment plans accordingly.
- Deliver treatment room services including vaccinations, cervical screening and wound care (depending on competencies).
- Maintain accurate and contemporaneous patient records using SystmOne.
- Participate in clinical audits, quality improvement projects and service development.
- Work collaboratively with GPs, Clinical Pharmacists and the wider multidisciplinary team.

Job Purpose
The post holder will work as an autonomous practitioner within their scope of practice, providing high-quality nursing care to patients across Crofton and Sharlston Medical Practice.
The role will have a particular focus on the management of patients with diabetes, supporting the practice in delivering high standards of chronic disease management, preventative healthcare and health promotion. The successful candidate will hold an Independent Prescribing qualification and will be responsible for assessing, diagnosing, treating and managing patients appropriately within their competence.
The post holder will work collaboratively with GPs, Practice Nurses, Healthcare Assistants, Clinical Pharmacists and other members of the multidisciplinary team to deliver safe, effective and patient-centred care.
Key Responsibilities
Diabetes Management
Lead and manage diabetes clinics.
Undertake annual and interim diabetes reviews.
Assess, diagnose and manage patients with Type 1, Type 2 and pre-diabetes.
Review and interpret pathology results, including HbA1c, renal function and lipid profiles.
Optimise diabetic medication regimes in accordance with local and national guidance.
Initiate and titrate diabetes medications within scope of practice.
Provide education and support to patients to encourage self-management.
Refer appropriately to specialist diabetes services where required.
Support achievement of Quality and Outcomes Framework (QOF) targets relating to diabetes care.
Prescribe safely, appropriately and cost-effectively within professional competence.
Undertake clinical assessments and formulate treatment plans.
Monitor effectiveness of treatment and review medication as necessary.
Work within national prescribing guidance and local formularies.
Maintain competence through regular CPD and prescribing audits.
Long-Term Condition Management
