We are looking for a warm, caring individual to fill the position of Team Leader in the Horsforth area of Leeds. Our services provide support to adults with autism and learning disabilities through a person-centred approach, including one-to-one assistance to achieve their goals, and access social activities and resources within their community. You will work as part of the senior team across two sites in Horsforth to ensure well-coordinated service provision. You will lead, coordinate, and supervise a team of support workers to achieve the highest standards of practice, customer care, and positive outcomes. In addition, you will be expected to carry out direct support with service users, work professionally alongside the multi-disciplinary team, and work closely in liaison with families and carers.
Responsibilities
- Providing practical and emotional support to adults with Autism and Learning Disabilities, including personal care and the provision of medication.
- Maximising service user outcomes by promoting independence and social inclusion.
- Encouraging service users to be involved in decisions regarding their care and lives.
- Collaborate with service users, their families and involved professionals to provide a consistent and coordinated service.
- Develop risk management plans in line with MDT and Mental Health & Learning Disabilities Services.
- Maximise and develop communication plans for services users.
Qualifications
- Previous experience in managing a team, and a minimum of 2 years in providing support to individuals with a learning disability, and/or autism, is essential.
- You will have a caring nature, and a commitment to making a positive difference in people's lives.
- You must also be willing to work flexibly according to the needs of the service.
