Overview: The Bury Intensive Support Service has been developed to provide a bespoke supported living service for adults over the age of 18, with learning disabilities, autism, and mental health and complex needs. The adults accepted into the service have been identified through the Transforming Care Agenda. This service operates 24/7, using a trauma‑informed and case‑management model of care, to provide support and opportunities for increased independence for three adults.

Key Responsibilities: We have developed a new and exciting role of Support Coordinator to provide operational leadership of the service in a sensitive and dynamic way. Staff members recruited will initially be required to work between our Registered Office, the service, and current care provision to ensure our service users experience a well‑planned transition into their new home.

You will support the Service Manager in leading the team to deliver the highest standards of support, enabling service users to develop and enjoy wellbeing, quality of life, and community connections. You will promote and reinforce a culture of responsive, person‑centred practice, and active support across the service. Strong working relationships will be built based on respect and unconditional positive regard. You will hold a caseload of, at least, one of the three individuals living in the service, with a clear focus on coordinating all aspects of person‑centred support and record‑keeping.

Eligibility: We can only accept applications from candidates who are located in and eligible to work within the UK.
