We are seeking an experienced practitioner to join our service as a Support Coordinator for our high quality care and support for individuals with learning disabilities, physical disabilities, and additional mental health support needs to live exciting and independent lives.

You will supervise an established team of staff providing person centered support, assisting with the coordination and delivery of services.
