An exciting opportunity has arisen to join our not-for-profit organisation as a Mobilisation & Assessment Practitioner. The Practitioner will have a key role around supporting referrals, assessments and transitions (including as part of new service mobilisations). The role will work collaboratively with frontline managers, senior leaders and housing colleagues to prioritise workload, adding capacity, skill and expertise in key areas of challenge.

We are looking for an individual with excellent communication, leadership, and IT skills, along with a strong ability to manage workloads efficiently. You will undertake duties such as supporting the mobilisation of new services, co-ordinating national referrals, providing support with void management and undertaking assessments.

Our ideal candidate will:

  • Have a minimum of 2 years experience in undertaking assessments and developing support plans
  • Have a professional qualification (e.g., RNLD, RMN, DipSW, DipOT) or Social Care Qualification (e.g. Diploma Level 5)
  • Create positive relationships with all stakeholders, people we support and families
  • Produce high quality documentation within concise timescales
  • Show initiative and problem-solving skills and find compromises to meet challenges
  • Willingness to travel and work flexibly
  • Be committed to continual learning and professional development

This position is available full-time or part-time (including school hours) with a minimum of 22.5 hours. Our head office in Stockport is conveniently located just 5 minutes from Stockport train station, with excellent transport links.
