An exciting opportunity has arisen to join our not-for-profit organisation as a Mobilisation & Assessment Practitioner. The Practitioner will have a key role around supporting referrals, assessments and transitions (including as part of new service mobilisations).

The role will work collaboratively with frontline managers, senior leaders and housing colleagues to prioritise workload.
