Creative Support’s Recovery Service in Whalley Range, Manchester is seeking an experienced Mental Health Practitioner to lead a team and deliver person-centred care for 11 individuals with enduring mental health needs and one with a learning disability.
You will conduct holistic assessments, develop support plans, monitor progress, and ensure safe medicines administration, using ECCO.
You will also support the Service Manager and supervise staff, with rota-based shifts.
