Creative Support is seeking an experienced Mental Health Practitioner to lead the Recovery Service in Whalley Range, Manchester.

You will oversee a small team delivering person-centred care to 11 individuals with enduring mental health needs and one with a learning disability, focusing on independent living and an active lifestyle.

You will conduct holistic assessments, create support plans, manage records on the ECCO system, and help coordinate with families and health professionals.
