We are seeking a Discharge Support Worker to join our Mental Health Floating Support team within wards across Greater Manchester. Multiple sites are available, including local community outreach in North and South Manchester.

This role offers a highly responsive hospital in‑reach service, delivering housing‑related support to people with mental health needs who face barriers to discharge, such as homelessness, a history of offences, and financial issues. Our person‑centred approach encourages service users in their aspirations while supporting them to achieve outcomes in independent living, social skills, community links, social inclusion, and employment pathways.

Responsibilities

  • Carry out person‑centred assessments to identify housing support needs
  • Set and monitor clear targets for outputs, timelines, and quality
  • Provide direct support and practical assistance in accessing housing and other support options
  • Support maximising financial wellness
  • Create links with housing and support providers to facilitate sustainable pathways
  • Ensure smooth transitions out of the hospital

Qualifications

Experience supporting individuals with mental health needs is an essential requirement.
