We are looking for a warm, caring individual to fill the position of Team Leader in the Horsforth area of Leeds. Our services provide support to adults with autism and learning disabilities through a person-centred approach, including one-to-one assistance to achieve their goals, and access social activities and resources within their community.
Responsibilities
- Providing practical and emotional support to adults with Autism and Learning Disabilities, including personal care and the provision of medication.
- Maximising service user outcomes by promoting independence and social inclusion.
- Encouraging service users to be involved in decisions regarding their care and lives.
- Collaborate with service users, their families and involved professionals to provide a consistent and coordinated service.
- Develop risk management plans in line with MDT and Mental Health & Learning Disabilities Services.
- Maximise and develop communication plans for services users.
Requirements
- Previous experience in managing a team, and a minimum of 2 years in providing support to individuals with a learning disability, and/or autism, is essential.
- You will have a caring nature, and a commitment to making a positive difference in people's lives.
- You must also be willing to work flexibly according to the needs of the service.
