You will support individuals to work towards achieving outcomes across key areas, including independent living and social skills, community links and social inclusion, and pathways to meaningful employment.
The role includes the following responsibilities: - Carrying out person-centred assessments to identify housing support needs - Ensuring clear targets for outputs, timescales and quality - Providing direct support and practical assistance in accessing housing and support options - Provide support to maximise financial wellness - Create links with housing and support providers to create sustainable pathways - Ensure smooth transitions out of hospital
Due to the nature of this role, experience in supporting individuals with mental health needs is an essential requirement.
