We are seeking an experienced practitioner to join our service as a Support Coordinator for our high quality care and support for individuals with learning disabilities, physical disabilities, and additional mental health support needs to live exciting and independent lives.

You will supervise an established team of staff providing person centred support, assisting with the coordination and delivery of high quality outcome-focussed support to tenants. You will be positive about supporting people to increase their independence and ensure that they are at the centre of all decisions made about their lives.

You will need to have prior relevant experience in social care and have an understanding of the needs of people with learning disabilities, physical disabilities, and additional mental health support needs. Your role will combine the provision of direct care and support with staff management and coordination of the team. You will need a positive and empowering approach with the ability to work collaboratively with staff, the families of service users, and other agencies and professionals.

You will be patient, professional and well organised with excellent communication skills, both verbal and written, along with a passion for quality and personalised service provision. It is essential that you have a minimum of NVQ3/H&SC Diploma Level 3 or equivalent and be willing to train to level 5 for the Support Coordinator position.

You will need to work flexibly as the post involves regular shifts, weekend working and occasional sleep-in duties (for which an allowance is paid). Full training is provided, as is the opportunity to work towards QCF Diploma in Health and Social Care.
