Are you a dedicated and passionate practice nurse ready to make an impact on patient care? Craneswater Group Practice is seeking to employ a Practice Nurse. This is an exciting opportunity where no two days are the same. This role is to assist the medical team in the care of our patient population to include treatment, preventative care, screening and patient education in the practice and local community. As an experienced nurse, you will join our proactive team, dedicated to offering health and wellbeing to our patients. Your primary focus will be health promotion and health prevention. This position's hours are negotiable with a minimum of 28 hours. This role will include extended hours with one late evening until 8pm.
Main duties
- To provide assessment, screening treatment services and health education advice.
- To organise and co-ordinate the provision of nursing services for the practice in liaison with other members of the nursing team and other Primary Health Care professionals.
- Provide nursing treatments to patients in participation with general practitioners or independently, to agreed guidelines and patient group directives.
- Provide general and specific health screenings to the patients with referral to general practitioners as necessary both in the practice and for our patients who are either housebound or living in Residential and Nursing Homes attached to the practice.

List size 11,100. Good CQC rating in all areas and high QOF achievement. We are part of the Portsmouth South coast PCN. We are a stone's throw away from Southsea seafront. Main purpose of the post
To assist medical personnel in the care of our patient population to include treatment, preventative care, screening and patient education in the practice and in the local community.

The Partners for clinical matters; The Practice Manager for administrative matters.
Major duties and responsibilities
- Providing assessment, screening treatment services and health education advice.
- Organise and co-ordinate the provision of nursing services for the practice in liaison with other members of the nursing team and other Primary Health Care professionals.
- Provide nursing treatments to patients in participation with general practitioners or independently, to agreed guidelines and patient group directives.
- Provide general and specific health screenings to the patients with referral to general practitioners as necessary both in the practice and for our patients who are either housebound or living in Residential and Nursing Homes attached to the practice.
Clinical knowledge and skills
- Wound care/removal of sutures and staples
- ECGs
- Venepuncture
- New patient medicals
- Immunisation routine, childhood and travel
- Women’s health cervical cytology, contraception
- Doppler and compression therapy
- Pathological specimens and investigatory procedures
- Undertake the collection of pathology specimens, bloods, swabs, smears etc. for daily collection
- Perform any investigatory procedures requested by the doctors
Treatment room supplies and equipment management
- In participation with the Office Manager and other members of the nursing team, ensure the maintenance of equipment and stock levels relating to patient care.
- Maintaining good order of the treatment room.
- Undertake the collection of pathology specimens, bloods, swabs, smears etc., for daily collection at mid-day.
- Perform any investigatory procedures requested by the doctors.
Patient data and records
- Ensure accurate details of all treatments (where applicable) are recorded in patients notes.
- Ensure accurate data entries are made on the computer record of any patient contact and admin issues, including consultations, telephone advice and clinical facts (e.g. blood pressures, peak flows, cervical status, vaccinations & immunisations).
- Ensure accurate completion and submission of all administrative forms which need forwarding to the Business Manager (e.g. Overtime sheets, holiday requests, study day requests).
- Ensure collection and maintenance of statistical information required for regular and ad hoc reports.
- Attend and participate in practice meetings as required.
- Maintain a notice board in waiting areas designated for patient health care/education as required.
- Ensure that any information relating to personal details is not discussed or divulged to any unauthorized person and is regarded as strictly confidential at all times.

Research and development
- Co-operate and participate as required in any research projects within the practice.
- Maintain effective liaison with partners, practice manager, staff and other agencies concerned with patient care and with all other disciplines within the practice, with appropriate regard to confidentiality.
- Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development.
- Engage with the internal annual appraisal process and maintain a comprehensive Personal Development Plan (PDP).
- Complete Mandatory Training on an annual basis.
Person specifications
- Qualifications: Registered general nurse with the NMC and maintains revalidation with NMC.
- Experience: Experience of working in primary care environment, clinical knowledge in wound care, ECGs, blood pressure, new patient medicals, immunisation, requesting pathology tests and processing results, collection of pathology specimens, bloods, swabs for daily collection, perform any investigatory procedures requested by the doctors.
- Ability to use own initiative and work well as part of a team.
- Experience of working as a practice nurse in a GP practice.
- Experience of System One.
- Nursing specialities: Women’s health (cervical screening, contraception, HRT).
