Summary
The Advanced Practitioner will play a crucial role in adult social care by providing high-level professional practice. This position involves supporting complex casework, safeguarding decision-making, and multidisciplinary discharge planning. The practitioner will work across hospital wards, community settings, and client homes to ensure safe, timely, and person-centered outcomes. They will also offer expert guidance to colleagues, lead on best practices, and uphold high standards of care in accordance with statutory duties, legislation, and organisational policies.
Responsibilities
- Lead on complex cases requiring advanced assessment, risk management, and intervention.
- Conduct holistic, strength-based assessments under the Care Act 2014, Mental Capacity Act 2005, and relevant legislation.
- Lead safeguarding enquiries and manage high-risk situations.
- Make defensible professional decisions regarding care planning, capacity assessments, and best-interest processes.
- Provide specialist advice to practitioners, health colleagues, and partner agencies.
Essential Experience Required
- Significant post-qualification experience within adult social care.
- Proven track record of managing complex cases and making advanced professional decisions.
- Experience in hospital social work or hospital discharge pathways.
Essential Qualifications Required
- Social Work qualification (e.g., BA Social Work, MA Social Work, DipSW, CQSW).
- Registration with Social Work England.
- Evidence of ongoing CPD relevant to adult social care practice.
Additional Information
- Working hours: 37 hours per week.
- Please note, we work on bi-weekly payments.
- Enhanced DBS with barred list required.
