Job Purpose:
SSW to work within Hayes and Five Elms Locality Team. The Locality teamwork is an ongoing care management intervention, and the coordination team leads on annual reviews of support plans, extra care housing support and direct payment work. This service also provides assessments under Care Act 2014 and work with young people transitioning from Children’s services to adult services, and beyond.
You will be supporting Younger Adults with physical disabilities and sensory impairment and Older People to live as independently as they are able and work with their carers to ensure risks are appropriately managed. You will need to be flexible, committed, and hard working with a willingness to work to quality standards. You will be working within a multidisciplinary framework and need to have good communication and negotiating skills. You will be working with Partner agencies and will be required to develop close working relationships with them.
Key Duties/Accountabilities:
- To promote and maintain independence for vulnerable adults in line with the wellbeing principles set out in the Care Act 2014 and the Health and Social Care Act 2022.
- To support carers to continue in their role.
- To protect adults at risk by robust assessments and the use of safeguarding procedures with an emphasis on making safeguarding personal.
- To assess the needs of Adults and their Carers in line with LB Bromley procedures based on the Care Act 2014 and other relevant legislation.
- With the Adult and Carer, formulate and implement personal budgets and support plans.
- Monitor, review and evaluate support plans including direct payments in order to ensure that they are consistent with eligible need.
- Provide supervision, support and advice to other team members and deputise for the Team Leader.
Essential Experience Required:
- Experience of working with adults of all ages including those approaching adulthood, those who substance misuse, have No Recourse to Public Funds, dementia, brain injuries, neurological and self-neglect/hoarding etc is essential.
- For qualified Social Workers/Nurses/Occupational Therapist registration with the (HCPC or NMC) is mandatory from 1 April 2005 is essential.

- Working hours: 36 hours per week.
- A satisfactory enhanced DBS disclosure is required.
