2-month contract role with a local authority, focused on coordinating and brokering care services for Adult Social Care residents with assessed needs. The position requires expertise in client groups, care systems, and local care markets to commission high-quality, cost-effective care. The role is crucial for ensuring timely hospital discharges in accordance with NHSE guidance and responding swiftly to community referrals to secure appropriate care settings for residents.

Responsibilities

  • Coordinate the sourcing, negotiation, and commissioning of care packages and placements for Adult Social Care service users.
  • Manage referrals for hospital discharges, ensuring all cases are processed within the required NHSE timeframe (within 3 hours of optimisation).
  • Liaise with hospitals, social workers, care providers, families, and other stakeholders to ensure seamless discharge planning.
  • Respond promptly to community referrals, identifying appropriate care settings that meet assessed needs.
  • Assess service availability, capacity, and cost to ensure value for money.

Essential Experience Required

  • Experience working within Adult Social Care, health, commissioning, or brokerage environments.
  • Knowledge of care markets, provider frameworks, and placement/purchasing processes.
  • Experience liaising with multidisciplinary teams, including hospitals and community services.

Essential Qualifications Required

  • Educated to GCSE level or equivalent (English and Maths essential).
  • NVQ Level 3 in Health and Social Care, Business Administration, or equivalent experience (desirable).

Additional Information

  • Working hours: 7.25 hours per day.
  • 1 day office attendance on Tuesdays and 4 days working from home.
  • Location: Waltham Forest Town Hall, Forest Road, Walthamstow, E17 4JF, United Kingdom.
  • Bi-weekly payments.
  • DBS required.
