The Care Home Trusted Assessor is crucial in minimising delayed transfers of care from acute settings. Collaborating with the Hospital Integrated Discharge Team, multidisciplinary discharge teams, and care home providers, the assessor evaluates patients ready for discharge into a care home setting. This role ensures timely, safe, and person-centred transfers, enhancing patient experience and supporting system-wide performance targets.

Responsibilities

  • Conduct assessments and re-assessments of hospitalised residents on behalf of care home providers ensuring compliance with agreed criteria and pathways.
  • Coordinate with hospitals, care homes, and community teams to streamline discharge processes and address barriers to timely discharge.
  • Verify discharge documentation and medication supporting initiatives like the Red Bag Scheme.
  • Effectively communicate complex and sensitive information with patients, families, and professionals utilising negotiation and reassurance skills.
  • Serve as a contact point for ward staff, MDTs, and care homes regarding hospital admissions and discharge progress.
  • Report on discharge quality issues and collaborate with stakeholders including CCG, Hospital Discharge Team, and Care Homes Forum.

Requirements

  • Proven experience in adult social care, hospital discharge planning, or care home settings.
  • Strong understanding of assessment processes and care planning for older people and those with complex needs.
  • Experience in multi-agency working and stakeholder engagement.
  • Ability to manage sensitive information and resolve complex discharge issues.
  • Relevant health or social care qualification (e.g. Nursing, Social Work, or equivalent).
  • Knowledge of legislation and best practices in adult social care and hospital discharge.
  • Enhanced DBS clearance required.
  • Driving license is essential.
