Overview

The Social Work Assistant in the Community Mental Health Team (CMHT) for Older People plays a crucial role in delivering comprehensive mental health services to older adults. This position focuses on enhancing the well-being and recovery of individuals through strengths-based, person-centered approaches. The role involves assessing and developing personalized care plans, managing a caseload, and providing direct support to promote independence and resilience. Collaboration with a multidisciplinary team is essential to implement effective mental health recovery strategies, working closely with social workers to achieve identified outcomes.

Responsibilities

  • Partner with individuals to identify their strengths, needs, and desired outcomes.
  • Support individuals in maximising independence and developing coping skills.
  • Conduct assessments, including well-being and risk assessments, considering outcomes, resources, policy, and legislation.
  • Recommend care and treatment plans to meet identified outcomes using available resources and support networks.
  • Collaborate with social workers to implement, monitor, and review care and support packages.
  • Participate in the team duty rota.
  • Maintain accurate social care records per the Adult Services case recording policy.
  • Manage and prioritise a caseload, providing direct support, advice, and attending relevant meetings.
  • Engage in supervision and report complex issues or safeguarding matters to supervisors or the Team Manager.
  • Develop professional knowledge and skills for quality work and personal development.
  • Participate in supervision, personnel review, team meetings, and service development meetings.
  • Understand and adhere to relevant legislation, guidance, policy, and procedures.
  • Follow the Council\’s Corporate Safeguarding Policy and report concerns regarding the safety and well-being of at-risk individuals.
  • Practice in a strengths-based and trauma-informed manner, attending training as available.
