We are excited to invite applications for the role of Discharge Coordinator within the developing West Midlands Learning Disability and Autism (WMLDA) Community Forensic Service (CFS). This is a unique opportunity to contribute to an innovative, regional initiative aimed at improving discharge planning and outcomes for adults with learning disabilities and/or autistic adults transitioning from secure inpatient settings.

This role is part of a two-year, non-recurrent project funded by the Reach Out Collaborative, with the contract commencing upon appointment. The post is available as a fixed-term contract or a two-year secondment opportunity for internal applicants.

The WMLDA Community Forensic Service is a new collaborative service delivered in partnership between Coventry and Warwickshire Partnership NHS Trust (CWPT) and Midlands Partnership University NHS Foundation Trust (MPFT).

We are looking for experienced, enthusiastic clinicians with expertise in community and/or forensic work with adults with learning disabilities and/or autistic adults. Applicants must currently be working at Band 7 or above.

The Discharge Coordinator will play a pivotal role in improving discharge pathways for individuals with learning disabilities and/or autistic adults across secure inpatient services in the West Midlands.

The post holder will work alongside the Reach Out Team, participating in system-wide calls, escalation meetings, and presenting data on clinically ready-for-discharge patients to Clinical and Operational Delivery Groups.

On-Call Duties

The role will include participation in an on-call rota, providing advice and clinical support outside of regular hours. On-call requirements will be confirmed and discussed further during the recruitment process.
