As a COP Nurse Assessor, your mission is to play a crucial role in ensuring the effective delivery of Continuing Healthcare (CHC) services to patients. You will be responsible for conducting comprehensive assessments and reviews of patients' health needs, applying the National Framework and guidance for CHC and adhering to the NMC Code of Professional Conduct. Your mission is to champion excellence in care, advocate for patients' rights, and facilitate the delivery of personalized care within the CHC framework. By utilizing your clinical expertise, compassion, and dedication, you will contribute to improving the health and well-being of individuals in our community.

Your expertise:

  • Conduct thorough and multi-disciplinary assessments of patients, ensuring compliance with standard operating policies and procedures.
  • Utilize your clinical knowledge and expertise to accurately determine patients' eligibility for Continuing Healthcare services.
  • Collaborate with the CHC team and relevant stakeholders to ensure the effective planning and management of patients' care needs.
  • Provide expert clinical advice and support to team members, patients, and senior leads, ensuring the delivery of high-quality care.
  • Act as a Duty Clinician when required, addressing urgent cases and ensuring timely responses.
  • Maintain accurate and up-to-date data systems, complying with the Data Protection Act 1998 and upholding confidentiality and data protection principles.
  • Participate in ongoing professional development and clinical supervision, adhering to NMC guidelines and maintaining your professional registration.
