As the Admin Team Leader for Continuing Healthcare (CHC), you will play a vital role in ensuring the efficient delivery of administrative services and support the overall effectiveness of the CHC placement team. Your mission is to lead and supervise a team of administrators, contributing to performance improvement, and delivering high-quality outcomes within CHC and the wider organization. By adhering to established policies, procedures, and protocols, you will champion excellence, promote the personalization agenda, and contribute to the successful delivery of Personal Health Budgets within CHC.

- Facilitate multi-disciplinary assessments and patient reviews, ensuring adherence to standard operating policies and procedures.
- Provide leadership and day-to-day supervision to the CHC placement team, driving performance improvement and ensuring quality outcomes.
- Collaborate with the Business Support Manager and stakeholders to plan and manage the CHC placement team's workstream effectively, highlighting opportunities for continuous improvement.
- Contribute to short, medium, and long-term business plans to achieve excellence and enhance the delivery of CHC services.
- Proactively support the CHC placement team in achieving compliance with key performance indicators and performance metrics.

- Manage and maintain patient filing systems accurately and robustly, ensuring efficient organization and access to essential information.
- Oversee the completion of all required duties within the CHC placement team, ensuring policies and guidance are followed appropriately.
- Uphold Information Governance policy and guidance, safeguarding the confidentiality and security of sensitive data.
- Collaborate with the CHC team to develop and implement project data collection systems, providing accurate and timely data for evaluation and improvement.
- Efficiently handle complaints, comments, and suggestions, resolving where possible and escalating as needed while providing support to the team.
- Contribute to the development of performance strategies and improvement programs to enhance the quality and effectiveness of CHC services.
- Effectively communicate information, risks, issues, and dependencies through reports and briefings to relevant stakeholders, progressing workstreams as required.
- Champion the personalization agenda across the organization, ensuring alignment with Personal Health Budgets delivery within CHC.
- Foster open communication and coordination with various organizations and individuals, promoting collaboration and teamwork.
- Supervise the CHC placement team, ensuring adherence to internal policies and procedures, and providing necessary support and guidance.
- Prepare correspondence and papers, as directed by the Line Manager, to support efficient communication within the department.
- Organize and facilitate department meetings, ensuring smooth and effective proceedings, including minute-taking and administrative support.
- Engage in relevant internal working groups, projects, and initiatives, providing valuable information and analytical advice.
- Gather, analyse, and update information to meet organizational needs, identifying best professional practices for continuous improvement.
- Provide coordination for relevant meetings, ensuring attendance reporting and providing valuable information and feedback.
- Investigate variance from plan/delivery targets and actively contribute to the implementation of effective solutions.
- Support the development of internal and external communications, maintaining regular contact with teams, stakeholders, and the Communications Team.
- Deliver against specified organizational objectives, achieving quality outcomes, and effectively prioritizing workload to meet deadlines.
- Support and inform the CHC team in resource targeting, monitoring, implementing, evaluating, and delivering plans through high-quality information and analysis.

- Undertake research and development activities as directed, promoting continuous growth and improvement.
- Ensure compliance with all statutory and mandatory training requirements, fostering a culture of continuous learning and development.
