As a COP Nurse Assessor, your mission is to play a crucial role in ensuring the effective delivery of Continuing Healthcare (CHC) services to patients. You will be responsible for conducting comprehensive assessments and reviews of patients' health needs, applying the National Framework and guidance for CHC and adhering to the NMC Code of Professional Conduct.

Your expertise:
- Conduct thorough and multi-disciplinary assessments of patients, ensuring compliance with standard operating policies and procedures.
- Utilize your clinical knowledge and expertise to accurately determine patients' eligibility for Continuing Healthcare services.
- Collaborate with the CHC team and relevant stakeholders to ensure the effective planning and management of patients' care needs.

- Act as a Duty Clinician when required, addressing urgent cases and ensuring timely responses.
