As the Nurse Practitioner in this role, your mission is to facilitate multi-disciplinary assessments and patient reviews in accordance with standard operating policies and procedures. You will adhere to the National Framework and guidance for Continuing Healthcare, as well as the NMC Code of Professional Conduct. Your expertise will be essential in ensuring the successful delivery of the workstream under the guidance of the Complex Case Manager/Clinical Commissioning Manager and the wider team.
Managing cases and providing end-to-end case management, including addressing issues related to funding, placements, invoices, and complaints, will be a key responsibility. Your role also involves following Continuing Healthcare guidance to maximize patient safety and supporting the organization in personalization efforts. Furthermore, you will play a pivotal role in the pathway delivery for Personal Health Budgets within NHS Continuing Healthcare.
Your expertise:
- Conducting face-to-face and virtual assessments/reviews as part of multi-disciplinary teams in alignment with standard operating policies and procedures.
- Assisting the Complex Case Manager/Clinical Commissioning Manager and the team in planning, managing, and delivering the workstream effectively.
- Managing cases as directed by the Management Team, ensuring seamless coordination and resolution of issues related to funding, placements, invoices, and complaints throughout the entire process.
- Ensuring compliance with Continuing Healthcare guidance to uphold patient safety and welfare.
- Supporting the organization in promoting personalization across services and contributing to the pathway delivery for Personal Health Budgets in NHS Continuing Healthcare.
- Identifying and arranging required services to meet the needs of individuals eligible for NHS Continuing Healthcare or healthcare elements of joint care packages.
- Conducting investigations into individual eligibility for NHS Continuing Healthcare in response to challenges or disputes.
- Acting as Duty Clinician as needed, assuming responsibility during specific periods.
- Maintaining accurate and up-to-date data systems in accordance with policies and the Data Protection Act 1998 and Code of Practice on confidentiality and data protection.
- Engaging in ongoing professional development and clinical supervision as per NMC guidelines to ensure continuous improvement and maintain professional registration.
- Providing cover for other Nurse Practitioners when necessary.
- Ensuring professional and appropriate communication at all times.
- Supporting the Clinical Assessment & Placement Team in achieving their goals and objectives.
- Building constructive relationships with internal and external partners/stakeholders.
- Communicating relevant information, risks, issues, and dependencies through briefings and reports.
- Collaborating closely with all care providers involved in delivering the service.
- Coordinating and participating in relevant meetings, providing advice, support, and information when requested.
- Reporting data collected as required and monitoring data processing and information flow.
- Offering specialist advice and guidance on functional and information matters related to CHC and associated services.
- Providing specialist associated services aligned with your skillset.
- Ensuring accurate and legible recording of information.
- Assisting and supporting individuals and their representatives throughout the NHS Continuing Healthcare process.
- Enhancing current management information reporting to improve decision‑making processes.
- Contributing to the development of performance and governance strategies, as well as improvement programs.
- Delivering against objectives and performance targets while achieving quality outcomes, effectively managing your workload within tight deadlines.
- Supporting training and induction of new staff.
- Undertaking research and development as directed.
- Carrying out any other appropriate delegated duties as required.
- Ensuring compliance with all statutory and mandatory training.
- Participating in assessments and reviews for patients with health funding, joint funding, or within your specialized area of expertise (e.g., ABI).
- Providing support in areas such as Disputes management, transitional bed process, SEND, and Safeguarding as needed.
