About The Team We Are Recruiting To

By collaborating with the Supporting Families Teams, Social Care Teams and a wide range of partners we will ensure that families receive the right help at the right time by the right people.

What is the job role?

To provide support to children, young people and their families through the delivery of a varied family support offer in order to improve outcomes for children aligned to the Supporting Families Programme. Your role will be to undertake Early Help assessments and act as Lead Practitioner, to assess, co‑ordinate, plan and deliver integrated and intense family support as part of a multi‑agency early help plan. You will be based in one of our Family Hubs.

In this role, you will work closely with partners across health, education, and the voluntary sector to deliver a whole‑family approach, supporting positive outcomes for children and young people.

You will engage with families from diverse backgrounds in a variety of settings, including family homes, schools, and community spaces.
