The predicted salary is between 33699 - 46142 £ per year.
The service will operate across 7 days, Monday – Friday 8.00am – 8.00 pm and Weekends 8.00am – 4.00 pm for which a shift allowance of 10% is paid. We have a Social Work post within our Hospital Community Discharge team. You will be supporting people living with a wide range of social care issues. There is a real emphasis on promoting independence and supporting safe and timely Hospital Discharge.
Our teams embrace strength-based practice, support Making Safeguarding Personal and work within key legislation such as the Care Act 2014 and the Mental Capacity Act 2005. You will need to have the confidence to advocate for clients and work in partnership with health colleagues, other statutory partners and voluntary sector providers. As a practitioner, you will need to have the ability to be creative, innovative and search for solutions that are not always obvious. Our teams are supportive and we will ensure you have the tools to do the job and to develop in your career.

- Social Work England Registration
- Working knowledge of and application of The Care Act 2014, Mental Capacity Act 2005 and relevant legislation.
- Experience or knowledge of safeguarding
- Organisation, recording and workload management skills
- Autonomy and decision-making skills
- Resilience and flexibility.
