Coventry City Council is seeking Social Workers registered with Social Work England to join the Hospital Community Discharge team. The role supports people with social care needs, promotes independence, and collaborates with health colleagues and voluntary sector partners.

You will work across a 7-day service with shift patterns and a 10% uplift, applying the Care Act 2014 and Mental Capacity Act 2005 to secure timely, safe discharges and promote person-centred care.
