Job Role

An exciting opportunity has arisen for a Community Case Worker within one of our One Coventry Integrated Teams. These teams are built around localities to coordinate urgent responses tailored to community needs, supporting hospital discharges and providing short-term reablement services. As no two cases are the same, you’ll enjoy a diverse caseload and have opportunities for personal development. Our teams pride themselves on a supportive environment and continuous professional growth. This is a great time to join us as we advance within the Local Integrated Teams with our health system partners. This post is exempted under the Rehabilitation of Offenders Act 1974 and appointment is conditional upon a satisfactory DBS check.

Candidate Profile

We seek someone who is compassionate, resilient under pressure, self-motivated, and a team player. You’ll work face-to-face with clients, collaborating with Social Workers, Nurses, Occupational Therapists, Physiotherapists, and other practitioners in a fast-paced, multidisciplinary environment. Excellent relationship-building skills are essential. You’ll work closely with team members across locations, including hospitals and care providers. Using a strengths-based approach, you’ll make confident decisions regarding rehabilitation, care planning, and promoting independence.
