Overview Go back Coventry and Warwickshire Partnership TrustPosition Patient Flow Co-OrdinatorThe closing date is 13 February 2026The main responsibilities of this post are to coordinate the delivery of clinical interventions for children and young people and their Families within the RISE Service. They will support specific processes for data management to contribute to the development of services, improve clinical outcomes and contribute to clinical and service audits. The post holder will have excellent organisational skills, effective communication skills and the ability to be innovative in regard to the way in which clinical interventions are coordinated. This role will be based between Whitestone clinic and Highfield House, Nuneaton so there is an expectation to travel. At times this role may also require travel across various CWPT sites as when required.Main duties of the job To respond to telephone queries with families who may be anxious, distressed or aggressive at times in a professional manner demonstrating empathy and sensitivity and being aware of own boundaries to re direct to senior staff.To respond to, record and report any adverse events or incidents relating to client care in an appropriate and timely manner.To respond to unpleasant or distressing conditions or behaviour (e.g. verbal abuse, aggression) in a professional manner, demonstrating empathy and sensitivity.To liaise with other members of the multi-disciplinary/multi-agency team, completing relevant forms and documentation to support referrals to the treatment waiting list with the required degree of urgency under the supervision of the Patient Flow Manager.To participate in the signposting process, contacting appropriate resources and/or support in order to facilitate effective discharge.To ensure that up to date written and/or electronic records and activity data are maintained in accordance with professional and Trust standards and policies.To support multi-disciplinary team meetings by completing and outgoing agendas, adding clinical notes of conversations to patient records and supporting patient flow onto the treatment waiting list. To ensure records around children and young people admitted to Tier 4 hospital are updated.
