An exciting opportunity has arisen for a Band 3 Health Care Assistant working as part of the intensive support within the Warwickshire locality, working with adults with learning disabilities in collaboration with families and other services to avoid specialist hospital admission. The post holder will work under the direction and supervision of a Qualified Nurse.
Experience of working with people with learning disabilities or within a community learning environment is essential. We are a hardworking, forward-thinking team looking for a motivated and capable individual to join us. You will be fully supported with training and supervision necessary for you to carry out this role.
The team's aim is to empower and promote individuals with a learning disability and complex needs to live meaningful lives. The role will entail working with adults with a learning disability with mental health needs to live in the community. The post holder will be part of a multi-disciplinary team providing support to people with Learning Disabilities who may present with challenging behaviours.

Provide holistic care for Adults with a Learning Disability and complex needs requiring specific health interventions. To be responsible for a caseload of service users who require close monitoring under the supervision of a Band 5 Community nurse. Be allocated to work with service users and assist in the development of care plans and risk assessments alongside the clinical team, including undertaking observations and monitoring. Provide a rich and stimulating environment enhancing the lifestyle of the service users and their individuality.
