The main responsibilities of this post are to coordinate the delivery of clinical interventions for children and young people and their families within the RISE Service. They will support specific processes for data management to contribute to the development of services, improve clinical outcomes and contribute to clinical and service audits.
Responsibilities
- Coordinate the delivery of clinical interventions for children and young people and their families within the RISE Service.
- Support specific processes for data management to contribute to the development of services, improve clinical outcomes and contribute to clinical and service audits.
- Travel between Whitestone clinic and Highfield House, Nuneaton; there is an expectation to travel.
- Respond to telephone queries with families who may be anxious, distressed or aggressive at times in a professional manner demonstrating empathy and sensitivity.
- Respond to, record and report any adverse events or incidents relating to client care in an appropriate and timely manner.
- Liaise with other members of the multi-disciplinary/multi-agency team, completing relevant forms and documentation to support referrals to the treatment waiting list.
- Participate in the signposting process, contacting appropriate resources and/or support in order to facilitate effective discharge.
- Ensure that up to date written and/or electronic records and activity data are maintained in accordance with professional and Trust standards and policies.
- Support multi-disciplinary team meetings by completing outgoing agendas, adding clinical notes of conversations to patient records and supporting patient flow onto the treatment waiting list.
- Ensure records around children and young people admitted to Tier 4 hospital are updated.
- Support patient flow with regards to offered interventions.
- Communicate effectively with professional colleagues and other agencies using a range of IT and AV media and equipment.
- Provide advice and information regarding assessment and intervention and communicate accurate information to others.
- Work to the standards, clinical pathways, protocols and policies within the department and organisation.
- Complete outcome and screening tools as required; ensure treatment waiting list forms are completed and discussions are followed up in meetings as needed.
- Actively participate in areas of development within best practice and governance activities relevant to specialist area.
- Ensure confidentiality of patient and staff information is maintained at all times.
- Participate in the personal development review system, ensuring that planned objectives are achieved.
- To follow and adhere to CWPT Health and Safety Policies and instructions.
- Safeguarding Children & Vulnerable Adults is your responsibility.
- To maintain up to date clinical records for children and young people in accordance with CWPT policy and local departmental protocol.
- Support in departmental audit/research activity as required.
- Combination of sitting, standing and walking; frequent need to be seated for a considerable amount of time at a computer.
- To independently write up clinical notes with accuracy and in a timely manner.
- To be aware of the sensitive nature of information that may be shared about the children and young people in our service.

- Personal information is confidential and disciplinary action will be taken if disclosed inappropriately.
