Intensive Support Team for Adults with Learning Disability

Part time / 20 month Secondment

We are excited to be able to offer a full-time band 6 nursing role within the Intensive Support Team. We are a community-based team that provides intensive assessment and interventions to individuals who are at risk of admission to hospital with the aim of avoiding unnecessary admissions, as well as supporting individuals through discharge transition out of hospital. We work in close partnership with individuals, their families and carers along with the local community learning disability teams, care providers, inpatient services and social care.

You would join existing nurses in a team where there is strong multi-disciplinary working and where the contribution of nursing is highly valued and you will be working across Coventry and Warwickshire. Our aim is to empower and promote individuals with a learning disability and complex needs to live meaningful lives. We adopt Positive Behaviour support approaches to reduce length of time in hospital and to support the prevention of inappropriate admissions or readmissions. This will be working within the home environment or appropriate care settings, alongside the wider Multi-disciplinary team.
