We are excited to offer a band 6 nursing role within the Intensive Support Team. The team is community‑based and provides intensive assessment and interventions to individuals who are at risk of admission to hospital, with the aim of avoiding unnecessary admissions and supporting individuals through the discharge transition out of hospital. You will work across Coventry and Warwickshire in close partnership with individuals, their families and carers, the local community learning disability teams, care providers, inpatient services and social care. The role involves strong multi‑disciplinary working and mentorship of staff and students.
Responsibilities
- Provide intensive assessment and interventions to individuals at risk of hospital admission.
- Support people with learning disability and/or autism who display challenging behaviours, including those with mental health conditions.
- Assess individuals with significant complex needs and/or challenging behaviours requiring individualised packages of care.
- Work within the home environment or appropriate care settings alongside the wider multi‑disciplinary team.
- Adopt Positive Behaviour Support approaches to reduce length of time in hospital and prevent inappropriate admissions or readmissions.
- Mentor staff and students.
